Transcript Request

The SIS accepts and processes transcript request for current students, alumni and associated schools and agencies at no fee.  A completed transcript request form and picture identification is required to process.  Students under 18 must be accompanied by a parent or a designated guardian. The SIS Department is located in the Library Buiilding at the Grand Rapids Public School's Administrative Offices at 1331 Franklin SE, Grand Rapids,  MI 49501.    

The SIS department also provides assistance with completion of social security and disablility forms. Please allow 7 business days for record request  processing. 

 Lunch Forms Information

The SIS Department works in collarboration with the GRPS Nutrition Services Department to admininister and support the student meals programs.
